CMS enrolls and maintains active enrollment only while the provider or supplier certifies, and CMS verifies, compliance with title XVIII and the Medicare regulations, with Federal and State licensure, certification and regulatory requirements for the services it furnishes, and with the requirement not to employ or contract with individuals or entities excluded from Federal health care programs or debarred by the General Services Administration. A provider or supplier furnishing ordered, certified, referred or prescribed Part A or B services, items or drugs, and a physician or eligible professional who orders, certifies, refers or prescribes them, must maintain written and electronic documentation of the orders, certifications, referrals, prescriptions and requests for payment, including the NPI of the ordering practitioner, for 7 years from the date of service and provide access to it on request of CMS or a contractor; failure is a revocation ground for up to 1 year per act of noncompliance.
