The laboratory must have a written or electronic request for patient testing from an authorized person, and may accept an oral request only if it solicits written or electronic authorisation within 30 days and keeps the authorisation or a record of its efforts to obtain it. The requisition must solicit the name and address or other suitable identifiers of the authorized person requesting the test and, where appropriate, of the individual responsible for using the results or of the submitting laboratory, including a contact for reporting imminently life-threatening or panic values; the patient's name or unique identifier; sex and age or date of birth; the tests to be performed; the specimen source where appropriate; the date and, where appropriate, time of collection; for Pap smears the last menstrual period and any previous abnormal report, treatment or biopsy; and any other information needed for accurate and timely testing, reporting and interpretation. A patient chart may serve as the requisition if it is available to the laboratory at testing and to CMS on request, and information transcribed or entered into a record system or laboratory information system must be entered accurately.
