Subpart J: facility administration for nonwaived testing (sections 493.1100 to 493.1105) – US Clinical Laboratory Improvement Amendments (CLIA) Regulations (42 CFR Part 493)

US Clinical Laboratory Improvement Amendments (CLIA) Regulations (42 CFR Part 493) 1103: Where transfusion services are provided, hold an approved transfusion service agreement, provide prompt ABO grouping, D typing, antibody detection, compatibility testing and transfusion reaction investigation through a CLIA-certified laboratory, store blood under appropriate conditions, positively identify the recipient, and investigate and report transfusion reactions

What does 1103 require?

A facility that provides transfusion services must document all transfusion-related activity and meet four requirements: a transfusion service agreement, reviewed and approved by the responsible parties, governing procurement, transfer and availability of blood and blood products; continuous provision of prompt ABO grouping, D (Rho) typing, unexpected antibody detection, compatibility testing and laboratory investigation of transfusion reactions through a CLIA-certified laboratory or one CMS accepts as equivalent; storage conditions, including temperature, appropriate to prevent deterioration wherever blood or products are kept outside a monitored refrigerator, with policies for positive identification of the recipient; and procedures to prevent transfusion reactions and to promptly identify, investigate and report reactions to the laboratory and, as appropriate, to federal and State authorities.
