Singapore AI Governance Framework AGT-2.2.2: Require human approval at significant checkpoints and audit that oversight stays effective

Agentic framework 2.2.2: define checkpoints needing human approval for high-stakes actions, irreversible actions (deleting data, sending communications, payments), outlier behaviour and user-defined thresholds; keep approval requests short, contextual and risk-flagged, with edit or written justification for complex or high-risk cases; counter automation bias and alert fatigue by tracking override rates and response times, spotting outlier reviewers, training overseers on failure modes (noting chain-of-thought may not be a faithful explanation) and ensuring domain expertise; complement with automated real-time monitoring and alerts, agents monitoring agents, and denying actions by default when approval infrastructure fails.
