OSHA Safety and Health Regulations for Construction (29 CFR Part 1926) 1926.417: 1926.417 Lockout and tagging of circuits

Controls that are to be deactivated while work is done on energized or de-energized equipment or circuits must be tagged. Equipment or circuits that have been de-energized must be made inoperative, and tags must be attached at every point from which that equipment or those circuits could be energized. Tags are placed so they plainly identify which equipment or circuits are under work. The section's text requires tagging and rendering de-energized equipment inoperative; it does not itself prescribe a written procedure, specific lock hardware or a set tag format, so evidence focuses on tags actually applied at all energizing points and on the means used to make the circuit inoperative.
