New Zealand Information Security Manual (NZISM) 7.2.17.C.02: 7.2.17.C.02 Reporting weaknesses, malfunctions and quantifying incidents

Agencies should: encourage personnel to notice security weaknesses in systems or services, or threats to them, whether observed or suspected, and to report them; set up and follow procedures for reporting malfunctions of systems, software or other items; put mechanisms in place so that the volumes, types and costs of malfunctions and information security incidents can be quantified and monitored; and deal with personnel who breach agency information security policies and procedures through training and, where justified, a formal disciplinary process.
