Develop individual plans at regular intervals based on assessing what the child and family need and expect and considering the group as a whole, and keep them as documented information. Appoint people responsible for preparing, implementing, coordinating, evaluating and reviewing plans. Plans cover general and specific objectives with indicators; activities, strategies and resources; who takes part and their responsibilities (child, family, staff, partners); deadlines; associated risks and responses; and review intervals. Involve the child and family, and retain evaluation and review results.
