The sponsor should ensure that design, conduct, processes and the information generated are of sufficient quality for reliable results, participant safety and sound decisions, and that processes comply with the protocol, related documents, regulation and ethics. It should set trial-specific criteria for classifying important protocol deviations (those that may significantly affect data completeness, accuracy or reliability or a participant's safety, rights or well-being), assess decisions for their impact and manage related risks, tailor oversight to the trial's complexity and risks (with selection and oversight of investigators and providers central and including QA and QC of their activities), and make sure issues are escalated and followed up in good time. It may set up an IDMC to review accumulating safety and efficacy data and recommend continuing, modifying or stopping, and an endpoint adjudication committee, usually blinded to treatment; such committees should have relevant expertise, managed conflicts of interest, written charters and documented decisions.
