Quality improvement is a habit, not a project. The practice names a person responsible for leading its quality improvement systems, who may also be the risk lead. Information about quality and safety, such as incident learnings, audit results and changes made, is shared with the whole team through meetings, newsletters or notice boards. The practice asks the team how well its improvement systems are working. Team members can describe concrete improvements the practice has made in the past three years, for example to recall systems, waiting times, record completeness or infection control, and the practice keeps a log of improvement activities and their results.
The graph holds this control, the 0 it maps to, and the evidence behind each claim, over MCP and REST.
