Core Standard 6 Information management – Australia RACGP Standards for General Practices (5th edition)

Australia RACGP Standards for General Practices (5th edition) C6.1: Patient identification: the practice uses at least three approved identifiers, such as name, date of birth, address, gender or a healthcare identifier, to match patients to their clinical information at every point where a mix-up could cause harm

What does C6.1 require?

Misidentification leads to wrong results, wrong medicines and privacy breaches. At registration, when retrieving a record, when giving results, when labelling specimens and when administering medicines or vaccines, the team confirms identity with a minimum of three approved identifiers, which may include the patient's full name, date of birth, address, gender, Medicare or Individual Healthcare Identifier; a room or appointment number is not an identifier. Staff ask the patient to state the details rather than reading them out for confirmation. The practice has a procedure and trains staff in it.
