Quality Improvement Standard 3 Clinical risk management – Australia RACGP Standards for After-hours and Medical Deputising Services (5th edition)

Australia RACGP Standards for After-hours and Medical Deputising Services (5th edition) QI3.1: Managing clinical risks: the service monitors, identifies and reports near misses and adverse events in clinical care, and the team improves its clinical risk management systems to prevent them recurring

What does QI3.1 require?

Every near miss is free information. The service has a system through which any team member, including sessional practitioners and call handlers, can report a clinical near miss or adverse event, such as a delayed visit, a missed deterioration, a medicine error or a report that never reached the practice, without blame. Reports are recorded, reviewed by the risk or quality lead, discussed by the clinical team, analysed for system causes and followed by improvements that are communicated and checked for effect; serious events are reported externally where the law or indemnity insurer requires.
