Core Standard 6 Information management – Australia RACGP Standards for After-hours and Medical Deputising Services (5th edition)

Australia RACGP Standards for After-hours and Medical Deputising Services (5th edition) C6.1: Patient identification: the service uses at least three approved identifiers, such as name, date of birth, address, gender or a healthcare identifier, to match patients to their clinical information at every point where a mix-up could cause harm

What does C6.1 require?

Misidentification is a particular risk when the patient is unknown to the clinician. At booking, on arrival at the home or clinic, when retrieving a record, when labelling specimens and when giving medicines or vaccines, the team confirms identity with a minimum of three approved identifiers, which may include full name, date of birth, address, gender, Medicare or Individual Healthcare Identifier; a booking number is not one. Staff ask the patient to state the details rather than confirming details read to them. The service has a procedure and trains staff in it.
