Part 4 Assessment and management of health conditions for Category 1 and 2 Safety Critical Workers – Australia National Standard for Health Assessment of Rail Safety Workers 2024

Australia National Standard for Health Assessment of Rail Safety Workers 2024 4.10: Psychiatric conditions: assess fitness individually on the specific pattern of illness, potential impairments and severity rather than diagnosis, for conditions including schizophrenia and other psychoses, bipolar disorder, depression, anxiety disorders, post-traumatic stress disorder and personality disorders, using tools such as the K10, considering medication effects, insight, adherence, the effects of rail work and critical incidents on mental health, and specialist or psychologist input, and apply the criteria in Table 20 with the review requirements

What does 4.10 require?

Safety Critical Work is a complex psychomotor performance depending on arousal, perception, learning, memory, attention, concentration, emotion, reflex speed, time estimation, auditory and visual function, decision-making and personality; train drivers must stay aware, perceive, interpret, anticipate and act on signals, sustain selective and divided attention and vigilance, memorise information, cope with emotional demands, low decision latitude and solitary work, and respond appropriately to emergencies. Psychiatric conditions encompass cognitive, emotional and behavioural disorders such as schizophrenia, depression, anxiety disorders and personality disorders, differing greatly in cause, symptoms, severity and whether episodic or persistent, and their impact varies with social circumstances, job and coping, so fitness assessment must be individualised on the specific pattern of illness, the potential impairments (Table 19) and severity rather than the diagnosis itself. Assessment uses history, tools such as the K10 questionnaire for anxiety and depression, consideration of medication effects, insight and adherence, and specialist psychiatric or psychological opinion where needed; it recognises that frontline rail work carries its own mental health risks, including exposure to suicides on the railway managed through critical incident programs, and that employee assistance referral may help. The fitness criteria in Table 20 set when a worker with a psychiatric condition is not Fit for Duty Unconditional and the conditions, including treatment, stability, insight and specialist review, under which Fit for Duty Subject to Review may apply for Category 1 and Category 2 work.
