Part 2 element 4 Assurance, monitoring and improvement (measures 4.1 to 4.4) – Australia Heavy Vehicle National Law Safety Management System Standard 2026

Australia Heavy Vehicle National Law Safety Management System Standard 2026 4.3: Continuous improvement: use information from monitoring, review, investigations and audits to decide whether improvement or corrective action is needed, act proportionately to risk, address underlying causes, target actions appropriately, implement them in practice, check their effectiveness, apply learning across the SMS, and keep the SMS suitable as risks, operations and knowledge change

What does 4.3 require?

The operator uses what monitoring, audits and incident reviews reveal to find improvements and to change the SMS where warranted, through planned, risk-based review and improvement including in response to incidents, operational change or emerging risk; change is not required for its own sake where no need has been identified, and no method or tool is prescribed. Eight things are looked for. Information from monitoring, review, investigations, audits and other assurance work is considered when deciding whether improvement or corrective action is needed. Decisions to act, and the action chosen, reflect the level of risk and the significance of the issue rather than a uniform response. Corrective actions go after the underlying causes of issues or control failures, not the symptoms. Improvement actions fit the issue and may change controls, procedures, systems, resourcing, competence or any other part of the SMS. Actions decided on are actually carried out rather than left as plans, recommendations or open findings. The operator checks whether an action taken has fixed the problem and improved safety or control performance. Lessons from issues, corrective actions and improvements are applied across the SMS so the same problem does not recur elsewhere. And the improvement arrangements keep the SMS suitable, effective and relevant as risks, operations and knowledge move on.
