Where there are reasonable grounds to believe a reportable situation has arisen for the licensee (a significant breach or likely breach of a core obligation, an investigation running more than 30 days and its outcome, gross negligence or serious fraud), the licensee lodges a report in the prescribed form with ASIC within 30 days after it first knows of, or is reckless as to, those reasonable grounds (offence and civil penalty). APRA-regulated licensees may satisfy this through a report to APRA containing the same information.
