An applicant must give a signed change statement if anything in the application or its documents changes before the decision (s 84). Every licence is subject to statutory conditions that the holder complies with information requests under s 83 and keeps the chief health officer informed under s 93, plus conditions prescribed by regulation (s 89), and to conditions the chief health officer imposes, for example on supervision, security of substances and premises, record keeping and conditions the Poisons Standard recommends (s 90), which may be changed after a 14-day comment period (s 91). Where the holder expects a change to anything in the licence or application (such as the supervising person, a close associate or influential person, or a structural change to premises), it must give written notice at least 7 days before and, if the licence details are affected, apply to amend it (ss 92, 93).
