Competent reviewers who took no part in the audit or testing examine the practice statement and updates, the approved test plan and reports, the Stage Two report and evidence, the indicators achieved and any nonconformities and corrective actions, and confirm that boundaries and contexts are correctly defined, all five core characteristics were assessed, indicators are reported at component and system level, local requirements are addressed, and all nonconformities are resolved or under an agreed plan; the outcome is documented and grounds the decision. Certification officers whom the conformity assessment body has authorised, and who are independent of auditors and analysts decide to grant certification, grant it subject to conditions, defer it pending corrective action or refuse it; decisions are recorded in the body's management system and the client is informed in writing with reasons for refusal or deferral.
